How do i find out if its working? Or know for sure?
I'm not sure other than see how the pieces all line up, but you already have it together.
ZZP made 2 separate manifolds for a reason!
I would hook the N* to the front valve cover and not use or worry about the tiny groove. Just cap the nipple on back of gen 5.

Ok ill hook it up to the front valve cover. Would the oil cover be okay also or does it have to actually be the valve cover?

So I am all kinds of confused! I am trying to determine exactly what needs to be done before I get too deep into this upgrade.
My current setup is a L36 w/ stock top swap. I will be doing a Gen V blower spaw with a Gen III IC (I believe its a stage 2 ZZP unit). I will be using a N* TB and LQ4 MAF. I feel that I can modify my IC to match the PCV holes on the rear.
That being said, what exactly do I have to do to obtain a properly functioning PCV system? ...My understanding is that I will only have to connect the nipple on the back of the Gen V to the nipple on the N*. Yes?
If you're going to modify the IC to allow for stock PCV function, then you are correct; connect the stock PCV nipple on the SC to your throttlebody, just like stock. Stockity-stock-stock.
What i did to make mines work was plug up the rear nipple....the ran a line from the nipple on my N* to the hole i tapped on my valve cover oil cap.

For a gen 5 supercharger with n*.
You need the zzp tiny groove manifold made for the Intercooler or just like in the original post connect front valve cover to the n* throttle body and cap the nipple on the back of gen 5 supercharger.
The front hole does not line up through the Intercooler that is where the tiny groove manifold comes in. The rear hole can be drilled in manifold.
Or you could always add a breather and disable the PCV.

Waking this up because I am doing this same crap right now and wanted to add some info. I am putting a Gen V blower on a ZZP intercooler designed for a Gen 3 and using my own modified L67 lower intake. It seems to me that with this setup, even if you have a ZZP lower intake with the tiny groove in it you cant run the front PCV through the intercooler plate and supercharger by using the normal Gen V PCV nipple on the back. Atleast not without some TIG welding, milling and drilling. There is a small section of the PCV passage on the bottom of the Gen V that is exposed to the opening in the intake which would seem to negate all PCV function and even possibly create a uncontrolled idle/internal vacuum leak of metered air. Also the upper hole would have to be redrilled 1/4" over as it is mostly blocked. I will be capping the nipple and venting from my N* to the front valve cover. It seems to be the easiest way.
The pic is with the plate pushed as far to the edge of the bolt holes as possible, to cover as much as possible, with the plate centered it is worse.

Where does it go to though? Between the maf and tb? Or to the supercharger?
I can't tell in his pics. Will this correct the front pcv working properly and not running a breather?

Yes, that will allow for proper front PCV operation with a breather. It will need to go between the Maf and the throttle body blade. I believe he's using the N* so it would go onto the vacuum port.
On my build, I'm running a breather, though I plan to switch over to what Anthony, or even just tap the VC. I drilled and tapped the TB and ran a vacuum line from there to the rear PCV on the Gen V
So something like this needs to be done for the front and rear? Or the zzp already has the rear?

Isn't a 2006 GP GT already supercharged? It is this top swapped NA?
Who's intercooler?
Stock LIM or came from vendor for intercoolers?
What throttle body?
