At 158,000, I'd think it's safe to replace the catalytic converter. If it's not your problem, it will be soon. Pretty sure they only last ~10 years (which, at 12,000 miles/year, would be 120,000 miles). You know the cat is completely toast when you smell rotten eggs from the exhaust, but it can be clogged long before that state.

Do you notice anything specifically with the way it runs that could help us give a better guess on what you need to do? Somebody mentioned cleaning the MAF sensor, I would do that if I was you, it really helps some cars. It's like $7 for a can of MAF sensor cleaner....If you have carb cleaner at home you might want to also consider cleaning the throttle body of all the buildup. Those are probably not the whole root of your issue, but it's a simple, quick place to start and it will make a little bit of a difference... somebody in your area with a scanner could pull some data off your car and tell you what's causing the problem, could be a variety of things, cat, O2 sensor, etc..
