Check fuses and maybe swap a relay or apply power straight to the fan to make sure it still works.
If you have having temps rise that high and fast when you aren't moving you have a coolant flow problem or similar. Perhaps the thermostat isn't working correctly or maybe even the water pump fins aren't pushing coolant quick enough. The other possible thing is the radiator is partly plugged and can't let coolant move.
I've had similar problems in several vehicles. In my old 1500 the previous owner had filled the radiator with stop leak, which caked up the t-stat and stopped the coolant. Pulled the t-stat and she was good to go. I eventually replaced the radiator and performed a full flush before dropping a new thermostat in, otherwise it would just clog again. Check your radiator hoses. They both should be warm/hot. If one is MUCH cooler than the other, that is an indicator of a clog.
