I have had this happen to me once but on a 4-barrel carbed, 7.4 litre V8 (my dually). never would have thought a fuel injected engine would be capable of that...
In my case I had been coasting with the engine off, sadly i had left it in gear (with the ignition/key off), which drew a bunch of fresh gas into the cylinders, and leeched into the exhaust. Upon reignition... bang. Melted all the aluminum parts in my carburetor too. Lol. For me 2 new mufflers and a carb rebuild. Dont know what the hell the deal is with yours but something similar maybe? Funny thing is seeing how much more catastrophic yours was for a V6 vs a V8. Mine had just popped the seems practically lol. Yours looks like it could be re engineered to be a lethal bomb of some kind.

I showed my grandpa these and he said hes never in 60 some years seen anything like that before.

I've shot fireballs out of both my old turbo Talon and back in the day in my Bronco II. When the Bronco shot the fireball it shut off and never restarted. When the Talon did it was loading up really bad and I was just trying to keep it from stalling when it shot a 4 foot fireball out of the exhaust.
Same thing happenend to my stock mufflers thought i was outflowing my exhaust.. Right... Then found out since my car was dumping fuel with the issues... I deemed that the problem
